“High Risk”

Individuals may be in need of a “Harm Reduction” service delivery approach that encourages the individual to choose to return to the home by allowing flexibility in care management practices. While addressing the individuals’ safety plan, the focus will be on relationship/trust-building, self-esteem building and the reduction of high-risk behaviors. Each person served has their own specialized plan to ensure consistency, this is very important when working with individuals with special needs as there is an increased need for structure and consistency. The specialized plans for each individual are very important when working with the younger children who reside within the homes, these services require attention to the individual developmental stages.
